To qualify for transportation, patients must be within a 30-mile radius of our Momentum offices. When scheduling an appointment, patients will not be given a specific pickup time. Instead, they will receive a call the day before their appointment to confirm and provide the pickup time. It is important to answer this call. PATIENT VISIT RESCHEDULE:
Please call our Momentum office as soon as you realize you will not be able to keep your appointment. We prefer all cancellations be made at least 24 hours prior to your scheduled appointment time. This will allow us to give another patient the opportunity to be seen.

BEFORE PROCEDURE:
Do not eat or drink anything after midnight the night before your procedure. This includes food, liquids, water, soda, coffee, gum, mints, candy, etc.
A driver is required to accompany you and must be present at check-in, unless we are providing transportation. Please note that if we are providing transportation, it is for the patient only.
If you are taking blood thinners (e.g., Heparin, Coumadin, Warfarin, Plavix) or antibiotics, or if you have a temperature over 100.4°F, please inform our staff before your procedure.
Dress comfortably and leave all jewelry and valuables at home.
AFTER PROCEDURE
After the procedure, go home and rest for at least 12 hours. Avoid any strenuous activities on the day of and the day after your procedure.
You may resume normal activities after two days.
Mild soreness at the injection site is normal and may last up to two days.
Contact our office if you experience increasing or severe pain, fever, or issues with bowel or urinary incontinence.

The length of a pain management procedure varies depending on the type of procedure and the individual.
Sedation is available for most procedures, which may require additional time for preparation and recovery. We will provide specific details, including the expected duration, before scheduling your procedure.

ChartSwap
As of March 2, 2020 Momentum Spine & Joint has contracted ChartSwap to process requests for the release of medical records (protected health information) and billing records.
All patient health care information at Momentum Spine & Joint is confidential. Therefore, medical records cannot be released to any person or organization without the consent of the patient or the patient’s legally authorized representative (unless authorized by law).
Medical Records
Request for the release of medical records (protected health information) must be submitted in writing and must contain all the elements required by law. Authorizations must also be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, a parent or legal guardian must sign the authorization.
