We are currently accepting new patients. Thank you for considering Momentum Spine & Joint.

When you come in for your initial visit, we will ask you to complete our New Patient Registration forms if you were unable to download them in advance to your appointment.

Please remember to bring:

• Valid photo ID

• Current list of medications

In an effort to respect the time of all our patients, our staff strives to stay on schedule. For patients who are delayed or arrive late for their appointment, every effort will be made to see you the same day. Please note that wait times may apply, or your appointment may need to be rescheduled.
